Often when applying for permits or requesting permission to shoot at public
or private locations, the owner or government entity will require special
language on the certificate of insurance with specific policy endorsements.
The language and endorsements often broaden the coverage of the policy and
will incur additional premium.
To assist filmmakers in understanding the often confusing insurance
language, we have posted the following City/Other Special Certificates Guide.
This continually updated guide will provide the insurance certificate and
endorsement requirements by city, state or permitting entity.

1.
One
ACORD 25 Certificate of Liability Insurance naming "FilmL.A., Inc.. and its
officers, agents and employees” as "Additional Insureds".
2.
"Additional Insured" endorsement = "FilmL.A., its Directors, Officers, and
Employees", except when applicant’s insurance contains a “blanket”
additional insured. In that case, the certificate of insurance must include
a statement that “FilmL.A. is additional insured by blanket endorsement.”
3.
Certificate must include a thirty-day notice of cancellation provision (10
days for non-payment of the certificate premium). It must be obtained from
an insurer admitted to do business in California or written through a
California-licensed broker.
4.
Total minimum Commercial General Liability coverage limits (per occurrence)
are as follows:
$ 500,000 for still photography only
$ 1,000,000 for motion capture (filming)
$ 5,000,000 for aircraft use

Total minimum Automobile Liability requirements vary depending on the
drivers and vehicles to be employed in the production:

Where
the permit applicant, or any of its representatives, agents or employees
will use personal vehicles in the filming performance, insurance
coverage must meet the minimum requirements as set forth in the
California Insurance Code section 16056 (currently $15,000/$30,000).

Where the permit applicant, or any of its representatives, agents or
employees will use, during the filming performance, a vehicle that must
be driven by an operator required to maintain a either a Class A or
Class B California Commercial Drivers License or a license with a
special endorsement or certificate per federal or CA State law,
insurance coverage of not less than $1,000,000 per accident is required.
6.
Proof of Workers Compensation coverage is required. Must include Employers’
Liability coverage with limits of not less than $1,000,000 per accident per
CA state law.

1.
ADDITIONAL INSURED.
The City of Long Beach, and its officials,
employees, agents, commissions, and volunteers are included as additional
insureds with respect to all claims, demands, causes of
action, damages, settlement, expenses and costs (including but not limited
to attorney’s fees and defense and investigation expenses), or loss or
liability of any kind or nature whatsoever arising out of, or in any manner
incident to, the operations, activities, or undertakings of the Named
Insured or any of the Named Insured’s employees, agents, or other persons
permitted by the Named Insured to be on City premises in connection with the
Special Event/City permit.
2.
PRIMARY AND
NONCONTRIBUTORY COVERAGE (Waiver of Subrogation). The insurance afforded by
this policy to the City, its officials, employees, agents,
commissions,
and volunteers is primary insurance. Any other insurance or self-insurance
maintained by the City, its officials, employees, agents, commissions, and
volunteers is in excess of this insurance and shall not contribute to it.
3.
SEVERABILITY
OF INTERESTS. The insurance afforded by this policy applies separately to
each insured seeking coverage or against whom a
claim is
made or suit is brought, subject to the insurer’s limit of liability.
4. CROSS
LIABILITY. The naming of more than one insured under this policy shall not,
for that reason alone, extinguish any rights of one insured against another,
subject to the Insurer’s limit of liability.
5.
CANCELLATION
NOTICE. The insurance afforded by this policy shall not be reduced in
coverage or limits (other than by payment of claims),
cancelled,
or otherwise terminated during the effective period of this endorsement
except after thirty (30) days’ prior written notice has been given to the
City (ten (10) days’ written notice for cancellation due to nonpayment of
premium). Notice shall be sent by certified mail to the address shown above,
