Certificates of Insurance
are used to provide vendors, locations and other interested parties
with proof of insurance. The Certificate of Insurance is also
used to name designated parties as "Additional Insured" or
"Loss Payee." These documents are available to those
who have already applied for and purchased insurance coverage for
their event or project.
After
completing the form below, please submit, and we will promptly issue the
certificates upon receipt. Sorry, blank Certificates of Insurance
cannot be issued.

Because of
the specialized nature of Special Event, Short Term Production, Annual
Equipment Floater, Event Cancellation, Annual Events, DICE/Annual
Productions, Errors & Omissions, Foreign Production, Movie Boat, Trade
Show and Wedding Cancellation Insurance, once any policy has been
purchased, the coverage becomes non-cancelable. The premiums for the
insurance policies are fully-earned. Therefore, once the
coverage has been bound, there can be no refund.
We understand fraud is an unfortunate risk
in doing business online. However, chargebacks due to any other reason
than fraud will be turned over to a collection service.
